  * First Guidance Committee Meeting Form
  * Second (and greater) Guidance Committee Meeting Form
  * Various forms necessary upon PhD completion

Finally there's a form from MSU that needs to be filled out in advance of your second meeting. This has a list of classes, etc. on it. You need to get this form from [[crossla3@msu.edu|Kim Crosslan]].
