Dan Alt has provided his thesis as a template that actually meets the graduate school's thesis requirements. He provides them as is, with no guarantees of any kind. Because the wiki will only let me upload some file extensions, I've had to append “.doc” to all of the filenames. Just remove these and you'll find the correct extensions.

• a sample chapter file (994 B, 8y ago, 183 downloads) I kept my individual chapters in separate files. Not required, but if you chose to do so, here's what the individual files should look like.
• bib style (18.65 KiB, 8y ago, 293 downloads) This was what I ended up using for the bibliography style file.
• class file (13.96 KiB, 8y ago, 451 downloads) Explicitly in violation of the language saying not to distribute or modify it. Don't care. If he doesn't want us screwing with it, he can keep up to date with the grad school.
